The short answer I suppose is that I do both.
The long one is that I always have an idea of what I want to do and what it could look like. So I use reference to help me realize that idea. You can use reference however you want, I tend to use it simply to know what an element looks like in relation to what Im drawing, not to find a piece to copy in so to speak but it all depends on what you want to achieve.
